I, the undersigned, hereby acknowledge and declare that:

I understand, agree and accept that there are certain risks associated with the purchase of such products, which may be of physical nature, and include injuries, adverse effects, and life-threatening medical consequences. I agree to assume the full risk of injuries, including death, damages or losses that I may sustain as a result of using such products. I agree to waive and relinquish any and all claims I may have arising out of, or connected with, said products, whether they relate to injuries, risks, adverse effects, and life-threatening medical conditions to others or myself.
I discharge, undertake not to sue, and hereby release FakeFace Aesthetics, its related companies, employees, representatives, agents, officers, and directors from any and all claims, costs, demands, causes of action, and liability of any kind whatsoever for personal injury, death, property damage or loss, negligence, breach of contract or breach of warranty that I may now or in the future have, known or unknown, which in any way results from or arises out of or in the course of my use of the products.
I hereby release, indemnify, and hold harmless FakeFace Aesthetics, its employees, contractors, agents, directors, shareholders, representatives, and individuals inside the facility from any and all claims, proceedings, litigation, damages, demands, actions, causes of action, or liability of any kind whatsoever arising out of or related to the products, including but not limited to claims for negligence, breach of contract, or breach of warranty.
I acknowledge that I have been given the opportunity to ask any questions that I may have about the products, and that any questions have been answered to my satisfaction. I certify that I am fully informed about the risks of the products and that I have had the opportunity to discuss any concerns with the staff at FakeFace Aesthetics.
I understand and agree that there is no guarantee of a successful outcome with respect to the products. I shall have no claim against FakeFace Aesthetics in this regard.
This Waiver and Liability Form benefits the FakeFace Aesthetics, its successors and assigns, and its related companies, employees, representatives, agents, officers, and directors. This Waiver and Liability Form binds me and my heirs and personal representatives.
I have read and voluntarily signed this Waiver and Liability Form. I have not been coerced to sign this in any manner whatsoever.
This Waiver and Liability Form shall be construed in a broad manner to provide a release and waiver in respect of all risks, whether known or unknown and to the maximum extent as permissible under the applicable laws.
